Call for Nominations for Model Practices

Purpose

The American College of Clinical Pharmacy believes that pharmacists should be recognized and valued as the preeminent health care professionals responsible for the use of medicines in the prevention and treatment of disease. A step toward achieving this vision is to describe, publish, and disseminate clinical pharmacists' "model practices." Nominees will be invited to prepare a 3-5 page description of a model practice in clinical care, education, or research. These models should also describe the outcome and/or clinical impact of the model practice, as well as the practice's sustainability and transferability to other individuals and/or sites.

Eligibility

ACCP members who have established model practice in clinical care, education, or research are eligible. The definition of a model practice in each area is defined below:

Patient Care

Original and innovative clinical programs that have demonstrated improvement in the quality of patient care or improvement in the effectiveness of the health care system. The program should be sustainable and potentially transferable to other settings.

Education

Original and innovative educational programs that 1) have demonstrated improvement in student/trainee motivation and learning and 2) have had an influence on education/training within an institution, regionally, or nationally. The program should be sustainable and potentially transferable to other settings.

Research

Original and innovative research that has demonstrated improvement in patient care (including translational and health services research) or pharmacy education/training (including pedagogical research focusing on student, resident, or fellow education). The research endeavor should be sustainable and potentially transferable to other settings.
